Major vac leak or soft fail fuel pump. Had similar issues and codes when my pump started to go before it finally died and left me stranded.

@E46Coupe - Fantastic mate as you can see. Bit of jigging required with getting the right fit but very happy with them :D

Sounds like it might be internal slipping in the trans (worn clutches). When they throw the code for output sensor it doesn't always mean its a electrical fault. If the trans ecu sees a gear engaged...
