how did you check the starter? the solenoid on a starter can be bad and still work so most people think that if it starts all of a sudden, the culprit can't be the starter but in "some" cases it is....

sounds like the fuel pressure regulator. the only thing that bugs me though is that you're only getting a misfire on cyc 01.
try swapping one of the ignition coils from one of the good "cylinder"...

did you check the fuel pump? is it priming consistently? if you're e is later than a '99 then the fuel pressure regulator is a separate unit, you might want to replace that as well if ever.
