You guys were right it was a bad rotor. The rotors had a grove in it and the shoes were worn. Not sure what happened but somehow there was friction going on to cause this as the rotorss and shoes...

yes i do need help. I am having a hard time getting the brake pad carrier off. The 16mm bolt is almost impossible to get off. I tried using a pipe as leverage and still not moving.
