The codes are always cylinders 3 & 6. I guess it would be possible to have two leaking injectors but I really don't think they are the problem.

I finally replaced the maf and no more cog and limp mode. So if your getting 0 volts or low voltage from the maf sensor then you will get the cog and limp mode.

I have a 2000 323i which sometimes lost reverse. The car ran fine in forward gears but when put in reverse about 50% of the time would rev and go nowhere. My car has the GM made 5L40E transmission....
