Sleeve nut on top, bolt from underneath. No washer needed as the sleeve nut is essentially a giant washer. The sleeve nut replaces the prongs.

Bring the plastic cover into home depot...

I would clean it up, then wrap the hose above in tissue paper with just enough tape to secure it...
