It appears that the Mossberg 500 or Remington 870 are the two models that people choose the most.

Only use: Secondary, or even third line of defense for home
Cost: I don't need or want a Benelli
Frequency of use: Hopefully never, but will take it to the range to practice.
Skill Level: None. Believe it or not, I've never used a shotgun
Desired Brand: Don't care

So let's do this, give me the rundown on what to get, including type of ammo and any "must have" accessories for a one stop shopping trip.

Shotgun Brand:
Specific Model:

I am not qualified to answer this in entirety, but I can offer some input.

Brand: Remington
Model: 870, 12 gauge, probably 18.5 in barrel. Any variant will do, but tactical/police are nice.
Accessories: Shell carrier, tube extension, maybe tac light. In a shotgun, the ammo in/on the gun is all you have normally, so increasing that as much as you practically can is a benefit.
Ammo: no idea what to recommend here

My basis for my selections is that my father has an 870 and my best friend has the mossberg 500. I have shot both and I feel the 870 is superior and has a smoother action. I like the ergonomics better as well.
