Is this loud? Does it just bolt on and fit a 99 323i sedan?

Can I see the rest of the exhaust, if you have more pictures. I can determin if it will fit by seeing the pipes.

My Ride: 2000 328ci --> SP
You should measure the pipe inlet because if it's too small then it'll just be a bottleneck and rather than allowing the the gases to exit easier it'll slow it down thus reducing HP. If it's a universal then most probably it'll fit 323's and 328's but not 325/330's.
