It's changed. There was probably a time when it was universally derogatory. Every time it was said, it meant that the person being called a ni**er was lowly, subhuman, unintelligent, property, and so on. It was also completely acceptable, because vast swaths of society felt that it was accurate. If someone grew up believing that blacks were lesser humans, they could call them ni**ers without the least bit of hate or contempt. And when you refer to rap/hip-hop, it's being used in a totally different meaning than the derogatory meaning. It's being used to convey a very positive meaning, interchangeable with friend, buddy, homeboy, etc.

So the real answer is that when it's used to label a black person in a negative (according to modern standards) way, it's far less acceptable than it ever was.

But because it's entered some people's vernacular, it's also used without thought and without malice, in which case it's generally acceptable, albeit not without controversy.
