Trying to help my neighbor out with his ride....
What is the hose I have highlighted in the pic below?
Where does it go?
What does it do?

It's not a hose. It's a Cable. You've highlighted two different hoses thinking they're one. The one on the right is the Battery Cable. The one on the left is a Wire Casing. They seem to be in their proper positions.

no...from the other end of the hose
battery cable goes straight and another cable/hose bends down into the side of the engine

Yeah, that's the Coil Harness. It goes into the DME Box in front of the driver.

Take that casing apart...and you will see exactly what's going on. It's really quite simple.

Smoke test should have been the first place to start. There's a maze of Vacuum lines. Coincidentally, none of these things are near Vacuum lines.

Get some carb cleaner and start spraying it around in sections when the idles rasies you have found your vac leak.

A couple I had where right on top of the intake boot where the F coupling is.

And the other was going to the sap there is a vac line running on the side of the block pass side going to the egr vavle on the block.

But carb cleaner is really your best bet very easy and you will find your leak fast.
